These forms are meant to standardize and simplify applications for General Education status. To apply, simply download and complete the appropriate form(s) from the list below. You will also need a copy of your course syllabus (PDF preferred) and a form G from the University Curriculum Committee (found here). Submit these documents to [email protected].
