Veteran's Class Registration Form

  • This form must be submitted every semester.
  • Needs to be Signed by your Advisor.

Requesting a Parent Letter

A “parent letter" is a letter from your Primary Institution to your Secondary Institution informing them that you have met with your advisor and are taking a course(s) that will transfer back to your program plan at your Primary Institution. A “parent letter” is required for your Secondary Institution to certify your course(s) so that you may use your VA educational benefits to cover the cost.

Please add the following information in your request by clicking the link below: 

  1. Your name and Ferris Student ID
  2. Your current Program at Ferris State University
  3. The college where you are taking the course per your request
  4. The course name, course number and number of credits per course at your secondary institution
  5. Who the request needs to be sent (name and email address)
  6. Copy your academic advisor email in the request

  • First time applicants only
  • Form completed on Vets. gov Website

22–1995 (PDF) — Request for Change of Program/Place of Training

  • Transferring benefits from another school or
  • Changes to your program of study
  • Complete form and turn into VA Certifying Official

Survivors’ & Dependents’ Educational Assistance

22–5490 — Application for Survivors' and Dependents' Educational Assistance

  • First time applicants only
  • From completed on Vets.gov Website

22–5495 — Request For Change of Program/Place of Training Survivors' and Dependents'

  • Transferring benefits from another school or
  • Changes to your program of study
  • Complete form and turn into VA Certifying Official
