Decision Forms

These forms are used by the General Education committees to evaluate course applications for General Education status. The completed forms will all be publicly available on the application status page.

 Subcommittee Decision Form - Used by each Competency subcommittee member.

 University General Education Committee Decision Form - Used by the UGEC during their deliberations.
