To add or remove authorization to your records under FERPA, access your Student Academic Dashboard in Ferris360, click the "FERPA Authorization to Release Information" link, and complete
the form. Upon degree completion, a student may submit the Graduation Transcript Request form to obtain one free transcript. To request a free transcript, complete and send
this form to [email protected]. If your legal name differs from the name on your student record, please complete the
Legal Name Change form and submit with required documentation in person or via mail to the Registrar’s Office. When requesting a Letter of Recommendation / Verbal Reference from faculty/staff,
students should complete this form to specify which FERPA Personally Identifiable Information may be discussed in the
reference process. This can include when students request a reference for admissions
at another institution, scholarship applications, or employment opportunities. Utilizing
this form is required when a reference includes any personally identifiable information
as outlined here. References that exclusively utilize directory level information do not require utilizing
this form, however, students marked confidential will still need to complete a release
form as their directory level information is protected. The form must be completed for each recipient that will receive the referral. Students
can choose which type of information is released during a referral, and can choose
whether or not to waive their right to review a copy of the letter or know about contents
of the verbal communication. Similarly, a student can waive their right at any time
to future disclosures of information, but it will not affect disclosures previously
made prior to the receipt of a written revocation request. Ferris State University participates in the Michigan Transfer Agreement (MTA) and is both a sending and receiving institution. This agreement facilitates the transfer
of general education requirements from one Michigan institution to another. Students
may complete the MTA as part of a degree program or as a stand-alone package. The
MTA consists of a minimum of 30 general education credit hours as identified by the
college or university. Ferris students pursuing the MTA should contact the registrar’s
office to request an MTA audit by sending the completed MTA Certification form to
[email protected] . Transcripts for Ferris students that meet the MTA will be updated to include the
“MTA Satisfied” on the transcript. Students may apply as a Guest student to other Michigan public universities or community
colleges to complete and transfer coursework back to Ferris State University. Guest
students should complete Part I of this form, then send it to [email protected] for completion of Part II. The Registrar’s Office will complete the form and submit
it to the desired institution. Students may request to be marked ‘confidential’ by completing the Non-Disclosure of Directory Information Form. This prevents information from being distributed to third-party requests. This also
requires student communication with university officials to occur in-person with a
photo-ID or via university provided student email. Additionally, contacts on your
FERPA release will not have access to your information. For additional questions,

If your Social Security Number has changed, please submit the Social Security Number Change form with a copy of your SSN card via mail or in-person to the Registrar's Office. may complete the Total Withdrawal / Cancellation Appeal up to the first day of class for the same semester the following year. Appeals may
result in change in total withdrawal / cancellation registration status and/or grades
awarded. Please complete this form with as much detail as possible and send to [email protected]. Appeals are reviewed weekly. Students will be notified of the outcome as soon as
