CHP - Applications for the Clinical Programs

Application Period for the Clinical Periods

All students are required to make a specific application for acceptance in the clinical program. This is a yearly application cycle for the upcoming academic year, dates are noted below.

In addition to these applications, you must have applied to Ferris State University prior to the application period below. Questions about this should be directed to the College of Health Professions, dean’s office

January 15th - 30th

Dental Hygiene, AAS
Diagnostic Medical Sonography, AAS
Medical Laboratory Technology, AAS
Medical Laboratory Science, BS
Nuclear Medicine Technology, BS
Radiography, AAS
Respiratory Care, AAS

March 15th - 30th

 Nursing, BS (BSN) Traditional

 

Submitting the Application

NOTE: COHP 100, 101, 102 will not affect qualification semester.  They are only required for the competitive application

Enrolled in classes SPRING 2020 @ FSU -

Use the application link below. Unofficial transcripts will be required with the application “if” you have transfer classes that do not appear on your unofficial transcript record in MyFSU.  You can submit a copy in (DOC, JPEG, JPG, PDF) formats.

Applications 

NOT ENROLLED SPRING 2020 –

You must have applied to FSU prior to the clinical program application.

You will be required to submit unofficial transcripts from other colleges with your clinical program application.

You will be required to submit this paper application, along with unofficial copies of college transcripts to the [email protected] email. 

Nursing Application

Optional for Ferris State Students who meet requirements: College of Health Professions Competitive Program Entry

Competitive seats area available for the following programs (Nursing, Dental Hygiene, Diagnostic Medical Sonography, Radiography).  Up to 25% of the seats accepted each year are available for competitive entry.

To qualify, a student must meet ALL the following criteria:

Have a current 3.70 GPA at Ferris State University

Completed all the qualifying courses for the clinical program

Completed all the qualifying courses with no repeats

Declared the clinical concentration by the semester prior to the application cycle

Completed 24 (passed) credit hours for: NURS, DHYG,

OR

Completed 12 (passed) credit hours for: RADI, DMS

Students are required to submit a 2020 application noted above. In addition, you must submit a competitive application to the College of Health Profession during the application cycle.  Only competitive applications require an application and this additional supplement.
