A. For first time protocols, obtain and complete all applicable sections of the Application for University Approval for the Use of Vertebrate Animals in Teaching and Or Research Version 1.0 (Protocol Application and All Applicable Appendices Pertaining to Your Project). Instructions are found within each section of the documents.
B. For renewal protocols, obtain and complete all applicable sections of the Application for University Approval for the Use of Vertebrate Animals in Teaching and Or Research Version 1.0 Renewal Only.
C. Once completed, PI will obtain ALL signatures requested on the signature page PRIOR to submission to the IACUC.
A. All IACUC approved Protocols will be forwarded to the IACUC Chair (or his/her designee) at which time the Chair or designee will apply the official IACUC APPROVED DOCUMENT, embossed seal to the signature page and will initial next to the seal.
B. Following application of the seal the document will be assigned a protocol number using the Protocol Number Format and will generate a PDF or equivalent electronic copy of the approved document. It will then be placed on the secure IACUC group website. The original will be filed with the IACUC.
C. All denied protocols will be filed according to federally mandated statutes and laws.
D. Any Protocols requiring changes before approval shall be returned to the PI with requested changes being indicated. Once the changes have been completed by the PI, he or she will return the document to the IACUC Chair at which point the Chair will review that the changes have been made and the document will follow steps A-B in the chain of custody.
E. All protocols will be kept on file at minimum one academic year after which time the documents will be forwarded to Ferris State University’s Records Management services, and will be placed on a document retention schedule that will follow Federal, State, and Local mandates for Animal Research Protocol document retention. Protocols related to scientifically historic findings or studies involving unique animal strains may be deemed of Ferris Historical Value and placed in the University Archives at the discretion of the IACUC or upon request of the PI.
