FSU Camp Medical Forms
The required forms below, which are in a PDF format, must be completed and returned along with registration materials for each respective camp you are attending. You will not be allowed to participate until ALL of these forms have been submitted.
*All camp participants must sign and return the Medical Report & Medical Authorization/Waiver Form along with the tubing waiver form (if needed).

These forms must be completed and signed by all camp participants.
- Tubing Waiver Form
In addition to the waiver form above, this form must also be completed and signed by anyone participating in a tennis and/or golf camp held at Ferris State University. Both of these camps may include a tubing trip.
