*All camp participants must sign and return the Medical History Report and Emergency/Medical Authorization & Waiver Form for Minor Participants along with the tubing waiver form (if needed).

• Medical History Report
  This form must be completed and signed by all camp participants.

• Emergency/Medical Authorization & Waiver Form for Minor Participants
  This form must be completed and signed by all participants for any Ferris State University camp, including all team camps.

• Tubing Waiver Form
  In addition to the waiver form above, this form must also be completed and signed by anyone participating in a tennis and/or golf camp held at Ferris State University. Both of these camps may include a tubing trip.
