Instructions for filling out the Application form:

• Fields marked with asterisk (*) are required
• You will need to click on the fill-in space in any input box before you can enter information.
• Once the cursor is positioned in a fill-in space the Tab and Shift-Tab key can be used to move from one input box to another.
• Some of the fill-in spaces will accept more information than is displayed on the screen, just keep typing. The arrow keys can be used to move back and forth in the space.
• Some parts of the form have drop-down menus that are activated by clicking on the little down arrow next to the fill-in space.
• Some parts of the form have radio buttons that are activated by clicking on them. Only one radio button in a group may be activated at a time.
• Before pressing the Submit button please check all information you have entered. If you want to Cancel, close the browser window without pressing Submit button.
• You will need to send in a Certification Statement. This statement is already included with the Printable Version.
• You will receive the confirmation of your submitted application from the Office of International Affairs via Email.
• Applicants to the College of Pharmacy should also fill out an International Application.

When will you hear about your acceptance?

We will notify you of your admission decision four to six weeks after we receive your application materials. Students who are accepted into the University will be sent a letter of acceptance and the I-20 (if seeking an F-1 student visa) or the DS-2019 (if seeking a J-1 student visa).
