AD/HD Report
A student diagnosed with AD/HD must have a comprehensive evaluation. A physician may make the diagnosis. Additional information for the diagnosis may come from experienced evaluators who administered an instrument specifically designed to diagnose AD/HD. A Physician statement of Long Term Disability will be accepted otherwise, the comprehensive evaluation report should include the following:

The Evaluator's credentials

How the condition affects the student's learning
|
Other Health Impairment means having limited strength, vitality, or alertness, including heightened alertness to environmental stimuli, that results in limited alertness with respect to the educational environment that a) is due to chronic or acute health problems such as … AD/HD and b) adversely affects a student's educational performance. A Determination of disability shall be based upon a comprehensive evaluation by a Multidisciplinary Evaluation Team, which shall include one of the following: a neurologist, a pediatrician, a family physician or any other approved physician. Psycho-Educational or Multidisciplinary Evaluation Report (MET) for Specific Learning Disability
A student diagnosed with a Specific Learning Disability must have a comprehensive evaluation. This should be administered during high school or within three years prior to enrolling at Ferris. The report should include the following:

Specific learning disability means a disorder in one or more of the basic psychological processes involved in understanding or in using language, spoken or written, which may manifest itself in an imperfect ability to listen, think, speak, read, write, spell, or to do mathematical calculations. See MI Rule R340.1713 or IDEA regulations. Also reference Section 504 of the Rehabilitation Act of 1973.
