To order Individual Membership Certificates:

Send an email to Tammy Hamilton at phialphainfo@etsu.edu with your chapter name in the subject line.

Be sure to include:

	School name
	Name(s) of students
	Name(s) of honorary members
	Dates to appear on certificates
	Mailing address to mail certificates

Submit one check or money order (You may include an optional $ 6.00 for priority delivery)
