Yes, if a student employee is injured on the job, the student and supervisor
must complete an
EMPLOYEE INCIDENT FORM
and submit the form to Human Resources, Prakken
Bldg. Room 150, as soon as possible.
NOTE: A copy of the EMPLOYEE INCIDENT FORM will also need to be sent to the
Health Center and Public Safety.
