Off-Campus/Consortium Forms

Each semester you must complete a Consortium Form. Please attach a schedule of the community college classes being taken.

The following requirements must be met by the student in order for Ferris State University to process financial aid:
1. Student must have either 56 credits accepted by their FSU academic program or an associate degree.
2. Student must be admitted into an approved bachelors degree program offered at the Ferris site.

If the student does not meet the requirements above, they should contact the community college for possible financial aid instead of applying for aid at Ferris. (Remember you can ONLY receive financial aid from one institution at a time.)
To receive Financial Aid:
  1. Apply for financial aid by filing the Free Application for Federal Student Aid (FAFSA). When filing the FAFSA you should list Ferris State University's Title IV school code (002260). By using this code Ferris will receive your FAFSA application electronically.
  2. If you are selected for the verification process, you will need to provide FSU's Office of Scholarships and Financial Aid with a Verification Worksheet and the appropriate federal 1040 tax forms.
Accepting your Financial Aid Award Online:
Step One: Log on to MyFSU
  1. Go to www.ferris.edu

  2. Select "MyFSU" icon

  3. Type in your UserName (Computing ID) and Password (Instructions to get your Username and Password are on the homepage)

Step Two: Go to "MyServices" Tab

  1. Select "Student Services" select "Financial Aid" select "Award"

  2. Select "Accept Award Offer" select "Aid Year" select "Submit"

To receive a Financial Aid Refund:
Refunds, if any, will be mailed to you when the following requirements are met:
  1. The financial aid has been awarded and accepted.
  2. The consortium information form has been approved.
  3. The necessary adjustments have been made to the financial aid based on actual scheduled classes.
If the above requirements are met, checks will be mailed after the community college refund period is over. After that time, checks are mailed on a weekly basis as the student's paperwork becomes completed and approved.
 

Note: Ferris State University is not responsible for your costs incurred at the community college. It is your responsibility to pay any fees billed to you by the community college.

 
Requirements for the Consortium program :
Ferris State University has financial aid consortium agreements with 16 colleges within the state of Michigan. The students who want to be involved in the consortium agreements must earn their bachelors degree from one of the Ferris State University consortium sites. The combined classes from the consortium site and the community college can be used in determining the student's financial aid eligibility. (You can NOT get financial aid from two different schools at the same time.)
