What form do I need?

The required form is based on two questions. Who purchased or owns the device? And, who will be paying for the service to connect it to Lotus Notes? Please review the chart below to determine which form to choose based on your answers to those two questions.

  I purchased my device

The University purchased my device
I will be paying for the service Personal MCD Form MCD Approval Form
The University will be paying for the service Departmental MCD Form
