Summary of Disability Documentation

Ferris State University provides reasonable accommodations for students with disabilities. If you require or plan to request services and/or accommodations because you have a disability, the following information should be sent to the Educational and Career Counseling Center. FSU will make final determination of whether appropriate and reasonable accommodations are warranted and can be provided to the individual.

ADHD/ADD

Physician’s Statement of Long-Term Disability OR a Comprehensive Evaluation Written Diagnostic Report, which includes: a clearly identified diagnosis, ADHD/ADD Behavior rating or evaluation outcomes, and accommodations. AND a completed Verification Form for Documentation of ADHD/ADD AND a completed ADD/ADHD Questionnaire.

 (Documentation must be dated within three years of the student’s request for accommodations and include a case history. It MUST identify the student’s current level of functioning relevant to the specific academic environment. Evaluation can be acquired by a Certified School Psychologist, Licensed Psychologist, Physician or others qualified to diagnose ADD/ADHD).

AUTISM SPECTRUM/PERVASIVE DEVELOPMENTAL DISORDER DISABILITIES

A recent Written Diagnostic Report /Psychiatric Evaluation from an appropriate professional OR Physicians Statement of Long Term Disability, AND a completed Verification Form for Documentation of Autism Spectrum/Pervasive Developmental Disability, AND a completed Autism Spectrum/Pervasive Developmental Disorder Questionnaire.

(Documentation must be dated within three years of the student’s request for services and include a case history. It MUST identify the student’s current level of functioning relevant to the specific academic environment. Evaluation can be acquired by a Licensed Psychiatrist, Certified School Psychologist, Licensed Psychologist, or others qualified to administer tests and/or diagnose Autism Spectrum/Pervasive Developmental Disorder Disabilities).

PHYSICAL DISABILITIES, GENERAL MEDICAL DISABILITIES

Physician’s Statement of Long-Term Disability AND a completed Physical, General Medical Questionnaire.

(Although some individuals have long-standing or permanent diagnoses, because of the changing manifestations of many physical and/or general medical disabilities, it is essential to provide recent and appropriate documentation from a qualified evaluator that is relevant to the specific academic environment. A student’s current level of functioning relevant to the specific academic environment, during the past three years, MUST be addressed in the Physician’s Statement of Long Term Disability). 

PSYCHIATRIC/PSYCHOLOGICAL DISABILITY

Physician’s Statement of Long-Term Disability OR a Comprehensive Multidisciplinary Evaluation Team (MET) Written Diagnostic Report (complete with Psychiatric Evaluation/Diagnosis) AND a completed Verification Form for Documentation of Psychiatric/Psychological Disability AND a completed Psychological/Psychiatric Questionnaire.

(Documentation must be dated within six months of the student’s request for services and include a case history. It MUST identify the student’s current level of functioning relevant to the specific academic environment. Evaluation can be acquired by a Certified School Psychologist, Licensed Psychologist, Clinical Social Worker LMSW, LPC or others qualified to administer tests and/or diagnose Psychiatric/Psychological Disabilities).

SENSORY DISABILITIES (Vision, Hearing, Speech)

Physician’s Statement of Long-Term Disability OR Speech, Hearing, Vision Evaluation Written Diagnostic Report conducted by a licensed vision, hearing, or speech professional AND a completed Physical, General Medical Questionnaire.

(Although some individuals have long-standing or permanent diagnoses, because of the changing manifestations of many sensory disabilities, it is essential to provide recent and appropriate documentation from a qualified evaluator that is relevant to the specific academic environment. A student’s current level of functioning, during the past three years, MUST be addressed in the Physician’s statement of Long Term Disability). 

SPECIFIC LEARNING DISABILITY

Written Psycho-Educational Diagnostic Report OR a Comprehensive Multidisciplinary Evaluation Team (MET) Written Diagnostic Report (complete with Psychiatric Evaluation/Diagnosis) and MUST include:

Cognitive Ability/Aptitude: Intelligence Tests, i.e., :WAIS or WISCIII Academic Achievement Test, i.e., : KTEA, WIAT, or Woodcock-Johnson Battery Information Processing, i.e. subtest sections of above, Detroit Test of Learning Aptitude AND a completed Specific Learning Disability Questionnaire.

(Documentation must be dated within five years of the student’s request for services and include a case history. It MUST identify the student’s current level of functioning relevant to the specific academic environment. Evaluation can be acquired by a Licensed Psychiatrist, Certified School Psychologist, Licensed Psychologist, or others qualified to administer tests and/or diagnose specific learning disabilities). This does NOT include IEP’s (Individualized Assessment Plans) or k-12 504 Plans.

TRAUMATIC BRAIN INJURY/ACQUIRED BRAIN IMPAIRMENT

A recent Written Diagnostic Report AND/OR a Physicians Statement of Long Term Disability AND a completed TBI/ABI Questionnaire.

(Although some individuals have long-standing or permanent diagnoses, because of the changing manifestations of many sensory disabilities, it is essential to provide recent and appropriate documentation from a qualified evaluator that is relevant to the specific academic environment. A student’s current level of functioning relevant to the specific academic environment, during the past three years, MUST be addressed in the Physician’s statement of Long Term Disability. Evaluation for documentation can be acquired by physicians, neurologists, licensed clinical, rehabilitation, and school psychologists; neurophysiologists and psychiatrists and/or others qualified to diagnose traumatic brain injury/acquired brain impairment.)
