Student Teaching Application and Forms

Student Teaching provides essential practical experience under guidance to supervising teachers and university supervisors in public school settings. This field experience requires completion of all prerequisites.

All Student Directed Teaching Candidates must submit all forms listed below:

1. Student Teaching Placement Process Form (Print and Submit)
2. Graduation Clearance Information  (Read and Request)
3. Application for Student Teaching (Save and Submit)
4. Professional & Personal Information Form  (Save & Submit)
5. Academic Summary (Save & Submit)
6. Professional Resume (Submit)
7. Three (3) Letters of Recommendation (submit) 
8. Criminal Background Check and Fingerprinting Instructions (Read)
   
   

  All Experienced Teacher Review (ETR) Candidates must submit all forms listed below:
                (Only for experienced teachers, currently teaching in their own classroom)

• Experienced Teacher Review (ETR) Application (Print & Submit) 
• Graduation Clearance Information  (Read and request)
