Please read the following points before using the electronic travel forms above:

1. If your are processing the form in hard copy, the request portions of the form (parts A & B) both need the travelers signature. Please don't overlook this.
2. Please note, you must use the 'Advances' section of the form for any prepayments you request. This will automatically deduct at the bottom of the 'Actual Travel Expenses' section.
3. Those travels with motor pool slips will need to be processed in hard copy format.
4. For non-consecutive travel dates you still need to submit a 'travel detail' form. If you are submitting your travel electronically, please attach that file too
