22–5490 — Application for Survivors' and Dependents' Educational Assistance
- First time applicants only
- From completed on Veterans Online Application Website (VONAPP)
22–5495 — Request For Change of Program/Place of Training Survivors' and Dependents'
- Transferring benefits from another school or
- Changes to your program of study
- Complete form and turn into VA Certifying Official
