Off-Campus/Consortium Forms
Deadline date for submitting the Spring 2010 Consortium Form is December 1, 2009.
(This is the preferred date for processing, however the form will be accepted at any time).

Each semester you must complete the online Consortium Form, listing your classes. For financial aid purposes, only classes at your approved community college site will be reviewed by the FSU advisor.

If any changes are made to your community college schedule, you must submit a new Consortium Form listing the revised class schedule. If the community college classes are not approved by the FSU advisor, they will not be included when determining your enrollment level for financial aid.

This link will take you to the MyFSU log- in screen. Log into MyFSU, click on the Academics & Services tab, find the Financial Aid Forms and Links heading, click on On-line Consortium Form.

If you prefer a printable form, please send an email request, including your name and student ID number, to cnsrtfinaid@ferris.edu.

 

To receive Financial Aid:
 
  1. Apply for financial aid by filing the Free Application for Federal Student Aid (FAFSA). When filing the FAFSA you should list Ferris State University's Title IV school code (002260). By using this code Ferris will receive your FAFSA application electronically.
  2. After Ferris receives your FAFSA, we will send you an award letter or a letter requesting additional information.
  3. As a consortium student, you will be awarded financial aid by Ferris State University. You may NOT receive aid from another institution during the same enrollment period.
Books:

The Lundberg Bookstore allows both online ordering and in-store purchases. You may choose to order your books online, have your FSU textbooks shipped to you and have the charges for the books added to your Ferris billing account. You may be able to charge up to $400 worth of books and supplies per semester.

If you owe a previous balance to the Lundberg Bookstore, you will not be allowed to charge your books.

Refund/Disbursement:
Consortium students will only receive refunds when the following requirements are met:
  1. Financial aid has been awarded and accepted, and all requirements are completed.
  2. The Consortium Form has been received and approved.
  3. Funds will not be released until after the 100% refund date at your specific community college site has passed.
Note: Ferris State University is not responsible for your costs incurred at the community college. It is your responsibility to pay any fees billed to you by the community college.
